Here's the publisher's page on how to cite various items in DynaMed in the AMA, APA & Vancouver styles:
https://dynamed.ebscohost.com/about/citing-information .

Their use of boldface and underlining the links, however, is unorthodox (APA Style does neither). See also our guide to citing information: http://researchguides.library.tufts.edu/content.php?pid=482439&sid=4476112 .

To garner a permanent link for a DynaMed topic, click the last (chain-link) icon in the right-hand column.  Use the date for the latest update as the publication date.

Here's an example:

DynaMed (2013, December 2). Chemotherapy for advanced or recurrent non-small cell lung cancer (NSCLC).  Ipswich MA: EBSCO.  Retrieved 12/9/2013, from
http://search.ebscohost.com/login.aspx?direct=true&db=dme&AN=900317&site=dynamed-live&scope=site .
